Summary AL amyloidosis (AL) and monoclonal gammopathy of renal significance (MGRS) are both paraprotein-associated diseases. Both entities based on a paraprotein produced by clonal plasma-cell population like in undetermined (MGUS) or B-cell low grade lymphoma. Per definition MGUS low-grade lymphoma do not require treatment. But rare cases the M‑gradient acts as “toxic” protein inducing severe multimodal organ damage MGRS.
